Presence, Trust, and Empathy: Preferred Characteristics of Psychedelic Carers

This study (2022) used thematic analysis to assess posts from people who use psychedelics (PWUP) on online discussion forums to gather information regarding their preferences/characteristics they value in carers in non-clinical research. Key characteristics include presence, trust and empathy.

Abstract

“Research into psychedelic-assisted psychotherapy (PAP) is rapidly increasing with best practice guidelines being continuously updated. Yet the key principles of such guidelines are based on evidence accumulated during earlier research into these treatments and trans-disciplinary psychotherapeutic knowledge. One area of practice-based evidence that has not been considered is that accumulated by people who use psychedelics (PWUP) outside of clinical contexts. We conducted a thematic analysis of 403 posts on two online discussion forums with the aim of gathering information regarding the preferences of PWUP for characteristics they value in their carers in non-clinical contexts. We found three key distinct characteristics that PWUP preferred among their carers: presence, trust, and empathy. The characteristics were often consistent with the existing literature on PAP, though at times extended current understandings of key characteristics of PAP therapists, with the notion of carers being able to “hold space” during the psychedelic experience seen as an integral component of effective therapeutic outcomes that deserves more attention. Data obtained from PWUP in nonclinical contexts can be used to triangulate existing principles contained within PAP treatment guidelines and identify areas for further investigation.”

Summary of Presence, Trust, and Empathy: Preferred Characteristics of Psychedelic Carers

Scientific investigation of psychedelics commenced with the isolation of mescaline around 125 years ago. Several therapeutic models of PAP are used, including logotherapy, cognitive-behavioral therapy, acceptance and commitment therapy, and motivational enhancement therapy.

The therapeutic effects of PAP appear to result from the combination of the neuropharmacological effects of serotonergic psychedelics and the subjective experience after their administration. The quality of the acute psychedelic experience predicts long-term effects. Best-practice approaches to PAP are required, but empirical research regarding prerequired characteristics of therapists and carers working with psychedelics is sparse, and the training itself has not been fully evaluated. People who use psychedelics may be a rich source of additional information regarding personal preferences and important qualities required of those supporting individuals. In clinical contexts, therapists use PAP for the treatment of psychiatric disorders. The therapist or co-therapist team conducts preparation, administration, and integration sessions, and takes extensive prescreening and safety precautions.

PWUP online tend to be around the age of 20 years, male, and either employed or attending college. They value the experimental as well as the spiritual components of using psychedelics recreationally, and offer social support and counseling to each other.
